RESUMO

OBJECTIVES: The objective of this study is to evaluate the longevity of two fiber post cementation strategies in a prospective, multicenter, non-inferiority, double-blind randomized controlled trial. METHODS: A total of 152 teeth, with adequate endodontic treatment and loss of coronal structure and bilateral simultaneous posterior occlusal contacts, were randomly allocated to receive glass fiber posts cemented with a conventional cementation strategy (CRC group: adhesive system + resin cement) (Adper Single Bond + RelyX ARC; 3 M-ESPE) or a self-adhesive cementation strategy (SRC group: self-adhesive resin cement; RelyX U100/U200; 3 M-ESPE). The patients were recalled annually for clinical and radiographical evaluation with a 93% recall rate (142 teeth, with 74 at CR groups and 68 at SRC group). The primary outcome was survival rate, considering the fiber post debonding (loss of retention). The secondary outcome included the success rate of the prosthetic treatment with crown debonding, post fracture, and tooth loss (not related to post failure). Both outcomes were evaluated annually. The Kaplan-Meier method and Cox regression with 95% confidence interval were used for the statistical analysis. RESULTS: For the primary outcome (failures directly related to fiber posts cementation strategy), there were 4 fiber post debondings (2 per group), 8 root fractures (3 for SRC group and 5 for CRC group), and one mixed failure (debonding combined with root fracture for CRC), with both strategies presenting similar survival rates (p = 0.331), with 88.9% for the CRC group and 90.9% for the SRC group. For the secondary outcome (failures not related to fiber post cementation strategies), there were 8 crown debondings, 3 post fractures, and 3 tooth losses, with no statistically difference between groups (p = 0.701), with 77% for SRC and 82% for CRC. CONCLUSION: Fiber post cementation strategies with conventional or self-adhesive resin cement presents similar tooth survival and success rates. TRIAL REGISTRATION: NCT01461239 CLINICAL RELEVANCE: Both adhesive cementation strategies led to high survival and success rates and are indicated for fiber post cementation, even after a long follow-up period (up to 106 months).

OBJECTIVES: To evaluate the longevity of metal-ceramic single crowns cemented onto resin composite prosthetic cores using a self-adhesive resin cement in a prospective clinical descriptive study. METHODS: A total of 152 teeth were endodontically treated and received resin composite prosthetic cores and metal-ceramic crowns cemented with a self-adhesive resin cement. The patients included in the sample were recalled for clinical and radiography evaluation in an up-to-106-month period after the final cementation procedures, with an average of 62 months of follow-up. 91.5% of the sample (142 teeth) were evaluated regarding the treatment survival rate, analyzed considering the loss of crown retention (crown debonding) and tooth loss as the primary outcome. In addition, post debonding, and root fracture occurrences were also recorded as secondary outcomes to evaluate the success rate of the prosthetic treatment. The aesthetic parameters were also evaluated according to the FDI criteria. The Kaplan-Meier method and Cox regression with 95% confidence interval were applied for the statistical analysis. RESULTS: Regarding the primary outcome, the metal-ceramic crowns cemented with self-adhesive resin cement presented a high survival rate (91.5%), with 8 crown debondings and 3 tooth losses (1 due to caries and 2 due to periodontal disease) occurring after the evaluation period. For secondary outcomes, 9 root fractures and 4 post debondings occurred, generating a success rate of 72%. All crowns had a score 1 on the FDI criteria, indicating that they were clinically excellent or very good regarding the aesthetic parameters. CONCLUSION: The metal-ceramic crowns luted with a self-adhesive resin cement presented a survival rate of 91.5% after an average of 62 months of follow-up. Furthermore, the restorations remained aesthetically satisfactory over time, without changes that would indicate prosthetic retreatment. A success rate of 72% was obtained considering the secondary outcome, mainly related to intraradicular retainer failures (root fractures or post debonding). CLINICAL SIGNIFICANCE: The self-adhesive resin cement is clinically indicated for cementation of metal-ceramic crowns onto resin composite prosthetic cores.

Aim: The aim of this study was to report a maxillary sinus lift surgery associated with the installation of an immediate implant associated with late loading in edge with 3.5 mm in height. Methods: The sinus lift surgery was performed using the lateral window technique, followed by the immediate installation of the implant and subsequent manufacture of a screw-retained single prosthesis. Results: Through the technique of breast lift, it was possible to promote local bone formation in a vertical direction, enabling local dental rehabilitation. Conclusions: Following a solid and meticulous planning, even in crests with less than 5 mm in height, a transcrestal technique can be used predictably with an adequate clinical and radiological outcome, giving patients excellent stability of the grafted material and excellent clinical results(AU)


Objetivos: O objetivo deste estudo foi relatar uma cirurgia de levantamento de seio maxilar associada a instalação de implante imediato associado a carga tardia em rebordo com 3,5 mm de altura. Métodos: A cirurgia de levantamento de seio foi realizada através da técnica de janela lateral, seguida da instalação imediata do implante e posterior confecção de prótese unitária aparafusada. Resultados: Através da técnica de levantamento de seio, foi possível promover uma neoformação óssea local no sentido vertical, possibilitando a reabilitação dentária local. Conclusões: Seguindo um planejamento sólido e meticuloso, mesmo em cristas com menos de 5 mm de altura, a técnica de janela lateral pode ser usada de forma previsível com um resultado clínico e radiológico adequado, dando aos pacientes excelente estabilidade do material enxertado e excelentes resultados clínicos(AU)

This study evaluated fracture resistance values and failure mode of weakened roots restored with cast post and core, fiber post and anatomical posts. In vitro studies, published in English, Portuguese and Spanish, that evaluated weakened roots restored with at least two of mentioned strategies were searched for in three databases MEDLINE, PubMed and Scopus until June 2017; there was no restriction regarding the publication year. Fracture resistance values were obtained, and the following comparisons: cast post and core vs. fiber post and vs. anatomical post and fiber post vs. anatomical post were performed. Meta-analyses were conducted using the random effects model to calculate the combined effect of the difference between the averages. Six articles were included in the study. There was no significant difference between the fracture resistance values, but more catastrophic failures were observed on cast post and core, and the included studies presented a low risk of bias for the most domains. It was concluded that weakened roots restored with the searched strategies seem to have similar fracture strength, but fiber and anatomic post decreases the possibility of catastrophic failure. Este estudo avaliou os valores de resistência à fratura e modo de falha de raízes fragilizadas restauradas com núcleos metálicos fundidos, pinos de fibra de vidro e pinos anatômicos. Foram pesquisados estudos in vitro, publicados em inglês, português e espanhol, que avaliaram raízes fragilizadas restauradas com pelo menos duas das estratégias citadas, em três diferentes bases de dados MEDLINE, PubMed e Scopus até junho de 2017; não houve restrição quanto ao ano de publicação. Os valores de resistência à fratura foram obtidos, e as seguintes comparações foram feitas: núcleo metálico fundido vs. pino de fibra de vidro e vs. pino anatômico e pino de fibra de vidro vs. pino anatômico. Meta-análises foram realizadas utilizando o modelo de efeitos randômicos para calcular o efeito combinado da diferença entre as médias. Seis artigos foram incluídos no estudo. Não houve diferença significativa entre os valores de resistência à fratura, mas foram observadas mais falhas catastróficas nos núcleos metálicos fundidos, e os estudos incluídos apresentaram um baixo risco de viés para a maioria dos domínios. Pode-se concluir que raízes fragilizadas, restauradas com as estratégias pesquisadas, parecem ter semelhante resistência à fratura, porém os pinos anatômicos e de fibra de vidro diminuem as possibilidades de falha catastrófica (AU)

A articulação temporomandibular (ATM) possibilita a mastigação, deglutição e a fonética, faz parte do sistema estomatognático, e é capaz de múltiplos movimentos. A DTM (disfunção temporomandibular) representa certas condições musculares e articulares na região crânio orofacial que podem causar problemas nos músculos da mastigação e na ATM e suas estruturas associadas, sendo a cefaleia e a dor orofacial alguns dos principais sintomas relatados pelos pacientes. Objetivo: analisar a relação da DTM à dor orofacial e cefaleia através de uma revisão narrativa. Métodos: para a realização deste estudo, foram feitas buscas nas bases de dados Scielo e Pubmed, utilizando as palavras chave: Dor Orofacial, Disfunção Temporomandibular, Articulação Temporomandibular, Dor de Cabeça e Cefaleia. Conclusão: a partir dos artigos analisados na presente revisão foi possível concluir que existe relação entre dor na região orofacial e cefaleia com a DTM, porém ainda não é claro o mecanismo dessa correlação, sendo necessários mais estudos para elucidar esse quadro clínico(AU)


The temporomandibular joint (TMJ) enables mastication, swallowing and phonetics, is part of the stomatognathic system, and is capable of multiple movements. DTM (temporomandibular dysfunction) represents certain muscular and articular conditions in the orofacial cranial region that can cause problems in the masticatory muscles and on TMJ and associated structures, with headache and orofacial two of the main symptoms pointed by patients. Objective: to analyze the relationship of TMD to orofacial pain and headache through a narrative review. Method: for this study, research was done in the databases Scielo and Pubmed, using the key words: Orofacial Pain, Temporomandibular Dysfunction, Temporomandibular Joint, Headache and Headache. Conclusion: from the articles analyzed in the present review it was possible to conclude that there is a relationship between orofacial pain and headache with TMD, but the mechanism of this correlation is not yet clear, and further studies are needed to elucidate this clinical case(AU)

A constante busca pelo sorriso étnico cultural harmônico está fazendo com que, cada vez mais, as pessoas procurem o tratamento odontológico. Assim, o presente estudo teve como objetivo avaliar a percepção de estética dental de odontólogos que estavam cursando diferentes especialidades, levando em consideração o quanto isso afeta na sua conduta clínica. A coleta de dados foi realizada por meio de um questionário, composta por 108 cirurgiões dentistas cursando diferentes áreas de especialização em uma escola de pós-graduação no ano de 2016, em que cada participante avaliou fotografias que possuíam cinco alterações frequentes na dentição anterior. Foi realizada a análise descritiva dos dados e o teste qui-quadrado para a análise quantitativa. Através dos resultados, verificou-se que fatores como idade, especialização prévia e tipo de especialização em andamento apresentaram relação estatisticamente significante na percepção estética entre as diferentes áreas de atuação da Odontologia. Desse modo, foi possível concluir que o sorriso considerado esteticamente mais agradável entre os entrevistados possuía as seguintes características: plano incisal intermediário entre o reto e o que segue a linha do sorriso, nível gengival harmônico, ausência de desvio de linha média, ausência de diastema e matiz A1 (AU).


The constant search for a harmonious cultural ethnic smile is causing more people to seek dental treatment. Thus, the present study aimed to evaluate the perception of dental aesthetics by dentists who were studying different specialties, taking into consideration how much this affects their clinical behavior. Data collection was performed through a questionnaire of 108 dentists attending different areas of specialization at a postgraduate school in 2016, in which each participant evaluated photographs that had five frequent changes in the previous dentition. Descriptive data analysis and chi-square test were performed for quantitative analysis. The results showed that factors such as age, previous specialization and type of specialization in progress had a statistically significant relationship in the aesthetic perception in different areas of Dentistry practice. So it was possible to conclude that the smile considered aesthetically more pleasant for the interviewers had the following characteristics: intermediate incisal plane between the straight and the smile line, gingival harmonic level, absence of midline deviation, absence of diastema, and A1 shade (AU).

Self-adhesive resin cements gained widespread due to their simplified technique compared to regular resin cement. However, there is lacking clinical evidence about the long-term behavior of this material. The aim of this prospective clinical trial was to assess the survival rates of metal-ceramic crowns cemented with self-adhesive resin cement up to six years. One hundred and twenty-nine subjects received 152 metal-ceramic crowns. The cementation procedures were standardized and performed by previously trained operators. The crowns were assessed as to primary outcome (debonding) and FDI criteria. Statistical analysis was performed using Kaplan-Meier statistics and descriptive analysis. Three failures occurred (debonding), resulting in a 97.6% survival rate. FDI criteria assessment resulted in scores 1 and 2 (acceptable clinical evaluation) for all surviving crowns. The use of self-adhesive resin cement is a feasible alternative for metal-ceramic crowns cementation, achieving high and adequate survival rates.

Modeling liquids/resins have been used to build up resin composite (RC) restorations, although there is a lack of information regarding their effects on the color stability of the latter. Therefore, the purpose of the present study was to evaluate the effects of the presence of modeling liquid between layers of RC and the finishing/polishing state of the material on color change in specimens exposed to red wine staining over time. Specimens were prepared by placing four increments (±0.5 mm thick) of RC (Filtek™ Z350 XT, 3M ESPE) into molds; half of which were prepared by applying modeling liquid (Scotchbond™ Multi-Purpose™ Adhesive, SBMP, 3M ESPE) between the layers of RC, whereas the other half were prepared without SBMP (control). Light-activation was performed after application of the final RC layer using a light-emitting diode (Radii, SDI) curing unit with an irradiance of 900 mW/cm2 for 20 s. Each group was divided according to the surface finishing protocol (n = 7): nothing (non-polished) or polishing with Sof-Lex™/diamond paste (polished). Initial colors of the specimens were evaluated with a digital spectrophotometer and the CIEL*a*b* color system. The specimens were stored in wine (37°C) for 12 months, and the color measurements were reassessed after 4, 6, and 12 months of storage. Scanning electron microscopy (SEM) analysis was performed at the end. Data were analyzed using ANOVA and Tukey's test (α = 5%). The presence of SBMP resulted in lower overall color change of the RC as compared with the control. The non-polished specimens exhibited a significantly higher color change than the polished specimens. SEM images corroborated the previous findings. In summary, the use of modeling liquid between layers of RC shows potential for application to reduce or delay the staining process of RC over time. Moreover, polishing is essential to provide increased color stability of the RC restoration.
